AUSI Divers who lose their card or want a damaged card replaced then: Forward a completed AUSI Replacement Certification Card Form to AUSI Head Office, together with a passport photograph, appropriate fee and the damaged card (if applicable).
Divers with cards from another agency can get an AUSI Certification by: Forwarding a completed AUSI Replacement Certification Card Form to AUSI Head Office, together with a passport photograph, appropriate fee and a photocopy of your current certification card.
